ASP Form to MS Access change

I have an asp based webpage form, that takes the info and inserts it into a ms access database. I need to insert 4 more questions with each answer being a radio button. You will need to insert this into the web based form and the appropriate fields in the ms access database. I will also need a confirmation email going to the person filling out the from. I cannot use CDONTs.
