I am looking for someone to create an interactive PDF form that a specific customer would use when placing orders for their uniforms fro 6 different regions throughout the USA.
There are multiple items that can be ordered and each item is custom embroidered with one or more logos on left chest, sleeves and neck yoke, or the front of a cap.
There are different logos for each region so I want to be able to place an image of the logo for that specific region and show the placement of the embroidery, left chest, right sleeve, left sleeve, back of neck yoke, front of cap. I would fill in this information before sending the order form to them.
I have provided an example of an excel order form that shows some of what I need, but more information is required because there are a variety of logos being used by different regions. And pricing is not constant but based on the size of the logos being used. I've also provided a sketch with some of the features I want on it.
The information will include my being able to fill in the product number, product name, sizes, and pricing - and the client would fill in quantity per size, color, and the form would give total and calculate 8% tax
The following features will be required:
* a place to enter a PO#
* a place for the date order was placed
* a place to indicate which logo or logos to embroider and placement of embroidery
Once filled in, a copy of the form will need to be able to be saved so that it can be printed or emailed
* Ship to, company name, address, and phone number
