Contact Referrals/Payment

We need to add an area to our site for contact referral. There will be one or two pages of information about the program, links to login or signup, and a link to download all info in pdf format. We will provide all banners, graphics, pdf docs, etc. Users that sign up will enter basic contact info (10 fields or so) and choose a password. This info will be emailed to them for verification along with program details. Users refer clients. Each client makes 3 payments. The user receives 20 percent of each payment. There is an admin section that allows someone to check a box when each payment is made and have the date be recorded automatically. We will provide a spreadsheet with roughly 80 case types and fees to be related by name and PK. The admin section will allow us to update case fees and record the date of these changes, in such a way that prior reports will remain historically accurate. The admin section will also allow the user to view and print the monthly statements for every user, described below. When someone logs in they can update their contact info, refer a client, click a link to our autoresponder course, or check two simple client reports. Refer a client simply allows them to enter contact info that will be related to them in a one to many. The 2 reports are very basic. The first simply list all the clients they have referred and the date that each of the 3 payments was made. The second details their monthly pay statement. This report will show the date and amount of every payment for each client over a given month, and a monthly total. They should be able to view this information for the current month and previous months and also have the option to print it out. We will provide FTP access to our site, all graphics, banner, autoresponder info, site text, and pdf documents. You will provide the rest. We need to have this fully deployed in 30 days.
