Write a simple iPhone application

I'm looking for a simple app for my business. This app will be for an IT support business whereby a user can log in with facebook, fill in a form and 'ask for help' by filling in a contact form. The app will consist of a few pages but essentially. It needs to look modern and professional so NO CHEAP LOOKING APPS PLEASE. - First screen is simple, logo displayed in full as a splash screen (picture in the background as wallpaper. - Second (and main function screen) has logo on the top left, again wallpaper in the background thus not to make it too dull looking and three buttons. GET SUPPORT, CONTACT US & OUR SERVICES. - Get support screen Simple data capture form with Name, email, tel, and a brief description of their issue (helpful hints in light grey in boxes until text is entered. Is it possible to pull the info from Facebook or have the option to do this? The capture form then gets emailed to an email address or sent back to me via database server I can setup at this end. - Services again, this is fairly simple and would just be a ‘jazzy’ about us page stating what our core services are then the addition of a contact us option. - Contact us three buttons, one to call us thus using their mobile apple device to place a call via their network carrier, I would like a chat option, are there any good applications you know of for live chat back to us/head office etc and finally a message/email us button All screens to have a ‘back button of some sort to main screen.
