Create mobile app prototype

Start-up company looking to design health related app prototype. The app will make correlations between the user's happiness and their personal behaviors. The app will ask the user to rate their happiness level 5 times per day on a 1-5 scale. The prototype will also allow users to input and store personal information related to 5 personal behaviors (hours of sleep, glasses of alcohol, minutes of physical activity, cups of caffeine, and hours of work). The app must also generate graphs that illustrate the correlation between their level of happiness with their personal behaviors.
