Magento - Build Table Rate Shipping Table using Amasty "Shipping Table Rates" Plugin

I need a shipping table created for the Amasty Shipping Table Rates plugin. It calculates the shipping price by looking at price, weight and destination. I have attached the data for the table. Here is a link to the plugin: [login to view URL] And here is a link to the user manual [login to view URL] The final table will be too big to be created by hand so will need to be created automatically from the input data (perhaps through an excel macro or through some other script such as Python). The output should be a .csv file that can be imported into the plugin. Also any source used to generate the file should be provided.
