Build complex sign up form on Joomla site

Hello -

I've set up a simple site aimed to provide users an online sign up process. I need help creating the forms themselves. I am open to using ChronosForms but I need the form to look like my mockups. I also need some form validation (such as insuring proper email and phone number formatting). When a user signs up, the data entered should be saved and a notification should be emailed to me.

The main site is http://www.ecowaze.com. The rest of the project specs are below.

RULES TO APPLY:

1. You will not get a reply from me UNLESS you send me a link to an online form you built. (Do not just send me 30 links and hope I find your form.)

2. The online form you created must have the complexity of my mockups (so look at my mockup before you give me a bid).

3. In your reply tell me what platform you would use to create my forms. (i.e. ChronosForms or something else)

4. I need this done in the next 48 hours, so you must be available to work full time now. I will include $30 on-time delivery milestone in what we agree to.

PROJECT SPECS

1. Make SEF work

2. sharpen image on home page (from pdf)

3. put title over home page image that says "Sample Bid Request Email"

4. Get rid of dash in front of <title> display

5. Have all <titles> end with " | Ecowaze"

6. Add nice/stylistic "Sign Up" button at bottom of home page

7. Add "Sign Up" menu item to main menu

8. Both sign up button and sign up menu item should lead to the same sign up form.

9. Set up vendor sign in. Use the template form I created at [url removed, login to view] Users should be able to open form, fill it out, hit submit and the info get stored.

10. When user fills out the form, a notification should be emailed to me.

11. The user's information should be secure, not accessible by anyone but me (the site admin).

12. When user submits the sign up, the user should be taken to a post-sign up page. It should look like the template I created at [url removed, login to view] When the user fills this page out, the information should also be stored.

13. After user fills out the post-sign up form, they should be taken to a Thank You page. It should look like the template I created at [url removed, login to view]

Note 1- The buttons and form elements should NOT have the boxy look of my template. I created these in MS Word. Please use modern Web 2.0 form elements that have rounded corners. ChronosForms and Jquery provide nice toolsets.

Note 2 - The star in a circle in my template means the form element is "required"
