We need an application form for new customers to apply to be a distributor for our business. The task will include 3 webpages. 1: A welcome page with details of the company and an apply now button 2: An application form defined by - [login to view URL] - with the same fields, layout and required fields 3: then a thanks for applying form. I would like it written in asp.net with the entered fields written into the database (fields supplied) and an email generated to alert us of the new applicant. We require bootstrap so that it can be used for all platforms. Layout of the form and the asp.net code is crucial for support purposes so I would like to see some sample code before I assign the task. Standard MS naming convention required. This is the first of many many screens which are required by our business.
