Should contain six area's with a detailed listing of services suggested, each service next to a check box, with 3 other fields next to a check box.
Should contain a price field next to the title of the service before the details.
Should contain fields at the bottom for customer & strat-e-gics signature of agreement.
Should contain an estimated completion date.
Should contain a payment detail area, pre-paid or Strat-e-gics credit account.
Should contain a disclaimer above the signature area stating the customer has read and agree's to the Terms & Conditions of Strat-e-gics.
All documents must be in pdf format.
Fax Cover sheet is attached, as expected. You will need to upload the company logo to the far left corner, & modify to a pdf format.
