Convert Wordpress Theme to Responsive HTML5/CSS

My client has a wordpress website with a custom theme with header, footer and right sidebar. Since content does not change. the client no longer wants to pay for wordpress updates and backups and wants to change to a simpler HTML5/CSS responsive site. How much to convert the existing theme to responsive HTML5/css, and also recreate the forms to work? Wordpress will be removed and replaced by your files. Current wordpress theme: Has a header (logo on left. Phone number and email on right). Right sidebar (image, text, image, text, days/hours of operation table, text) Footer has an image and text. Total of 6 pages - home (3 short paragraphs, 2 images), - about (3 paragraphs, 1 image), - products (1 paragraph, 1 image), - testimonials (1 paragraph, 5 block quotes), - schedule an appointment (2 paragraphs and a form with 10 text, 2 dropdown, date (MM-DD-YYYY US format), textbox and captcha - email error checking requested.) - contact (form with email, name, subject and comments). Colors: Background - white. Navigation menu - blue background, white color font. H1 green (exact colors can be copied from existing wordpress site) Logo symbol is in a circle shape. The company name extends to right and and 3 services underneath it. Hosting is on Godaddy. Well commented code using correct US English please. Source files will be required once completed. 2 different concepts should be created similar to existing wordpress theme design - show mobile and monitor designs for each concept. 2 rounds of design revisions should be included. 1 round of text revisions (only to testimonials) Actual site will be provided in private discussions.
