Alter PDF form (Need completed quickly)

We need a PDF to be edited for our Basketball event. The original form is here: [login to view URL] We need the project completed as quickly as possible. Hopefully in a few hours. The changes needed are: - Change the Divisions table at top to match the attached Excel file division table. - Change the logo, event dates, email, title and website to match the information on our website: play3on3 [dot] com (remove current logo, website, etc.) - Change the boxes on the right to match the content from the attached Excel file - Change the footer info to 'Make Checks payable to Play 3on3 - Send payment to Po Box 1443 Hilliard, OH 43026' - Change the Waiver to match the one on our website: [login to view URL] (towards the bottom of the form) - Remove the 5th player and add the message 'Each team must have at least 3 players to participate' - For each player add a new question "Check all levels of basketball in which you have competed." Have check boxes for each of the following: No Organized Experience, Rec Youth Team, Middle School, AAU-Travel, HS Freshman, HS JV, HS Varsity, Adult League, College, Pro The final should be an 8.5" x 11" PDF high res for printing.
