Convert Image to Adobe Acrobat PDF Fillable (Fire Dept Fire reports)

We currently have a paper-only method to do fire reports**, and we'd like to have this imported AS-IS with the sections filliable with check marks on some things and numbers on others. (This is optional) The project MUST be in PDF format, fillable, and able to be printable. The content must remain AS-IS. 1.) You must convert the image to readable format (I do not have the actual copy of the document, the original was lost) 2.) You must convert it to PDF fillable format. 3.) You almost MUST provide the original document (word or whatever) so I can make changes and import them in the future to a PDF fillable format. ** Fire reports are for fire departments. Once I accept you, due to sensitivity of the content, I will show you the images that need to be imported into Word et al then transformed into PDF. You can do this via "Office Imaging Tools" in MS Office.
