I have a website that is hosted on a unix server. I am trying to create a contact me page with 10-15 questions so people can send me their info.
Once a user clicks submit, I would like to get an email with all the information that was left on the page. Currently the page contains some text fields, combo boxes, and checkboxes.
After clicking submit, I would like to the user to be directed to a "Thank You" page.
Currently, when I click submit, I get the following error:

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

(No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
