Hello,
I have designed a new logo for my Business and now need a word template with a letter head created for it.

The word template will be for reports and should have three pages
1. Cover Page
2. Main Page
3. Normal Content Page (which could be used for 20 content pages in the document)

In general I am looking for a MS Word template design which is:
• Very professional looking
• Elegant and simple in design
• Uses the logo colours in some way
• In MS Word 2013 for PC
• Should have all the correct styles etc created so editing is easy. This can be done at the end if you are the winner.
• The font in the logo is Century Gothic with colours : Hex #27a9e1 and #134a8e
• I think the normal body text would be good in Arial but am open to suggestions
• Uses the best practice MS Word template design principles

Cover Page
• Should be colourful
• This needs to feature the logo in a fairly large way
• It should have some type of image(s) relating to the business of Speech Pathology (like some children learning, some speech bubbles, child talking etc. This could be as a background, a main image/ half the page/ a watermark, on the side of the page etc. It is up to you to decide the design.
• The images must be either copyright free, or able to be purchased by me from a stock image website.
• It should contain sample text which identifies the type of report and report details:
o SPEECH PATHOLOGY ASSESSMENT REPORT
o Name: Donald Duck
o Address: 1 Smith Street, Lane Cove, NSW, 2066
o Date of birth: 29.12.2009
o Current Age: 3 years 7 months
o Date of assessment: 19.04.2015
o Date of report: 21.04.2015
• It should contain the details of the business - website address, email address, physical address, and phone number. These details do not have to be too prominent

Main Page
• This could be used for documents without the cover page if needed, so the header should take roughly 15% of the page so it is reasonably prominent compared to the normal content page
• It should include the logo, physical address, website address, email address, phone number at the top of the page
• It should also contain sample text with a table and some headings

Normal content page
• Should contain only a very small header/footer so the majority of the page is for content.
• Should contain the page numbers
• Should contain perhaps a very small version of the logo
• Should contain the name of the company – “Sydney Speech Clinic” and website address
• Should contain the name of the report in either the header or footer
• The example content should contain a table also which is styled in a design (e.g. it might have a blue header the same colour as the logo.)


I have uploaded:
• The company logo as an AI file
• A word file with the details of the company (this is a template I started myself, but then figured out I could not do it – hence the reason I am posting this job)

What you would need to supply:
• A word document template with the three pages listed and example content
• A link to any images used so I can buy them
